When you join VillageCareMAX, a team of health care professionals will work closely with you to help you get the care you need. This team of health care professionals is made up of your primary care doctor, a social worker, a member service representative, your caregivers and your VillageCareMAX Care Manager.
Your VillageCareMAX team will work closely with your doctors, help set up appointments, arrange for your care, monitor your medications and guide you and your family toward the best quality health care options available.
Your dedicated Care Manager will visit you in your home as long as you are a member of VillageCareMAX. Working together, your care team will help you get the care you need so that you can live the life you want.
