Your dedicated Care Team is central to helping you stay at home and in your community for as long as possible.
How it works
Upon enrollment, you will be assigned to an interdisciplinary care team, made up of health care professionals, which is led by a dedicated Care Manager who coordinates all aspects of your care.
The Care Manager works closely with your primary care doctor, helps set up appointments, arranges for your care, monitors medications and guides you and your family toward the best quality health care options available.
Your Care Manager will work to create a personalized care plan, which will be updated every six months based on medical necessity:
- The care plan is a written description of your needed services and how they will be provided.
- The care plan includes measurable goals and objectives.
