VillageCareMAX in Your Community
VillageCareMAX is in your community, proudly working to keep our members healthy and vibrant. You'll find a warm welcome at any of our three community centers located throughout the city – in Brooklyn, Flushing, and the Bronx.
All three of our locations are open to VillageCareMAX members, as well as all individuals looking to find out about upcoming wellness events and social activities, plus get help with:
- Understanding your Medicare benefits
- Filling out forms
- Getting grocery access
- Enrolling in one of our health plans

Bronx Community Center
